If you need to update your credit card information, please do the following:
1. Click on the link here.
2. Enter name and email address.
3. Next is to verify that you agree with the terms in using electronic records and signature.
4. Fill in all information in the Credit Card Authorization form. All fields are required.
5. Click in the Sign Here found in the Signature part in the bottom of the form.
6. DocuSign will then insert your initials or digital signature in the form by clicking in the ADOPT AND SIGN button in the bottom part or the page.
